1. An application for grant of food hygiene and safety certificate
2. A lawful copy of business registration certificate
3. A written description of material foundations, equipment and tools to assure food hygiene and safety conditions consists of:
- Drawings of production facilities, business and the surrounding area ground;
- A written description of the manufacturing process (technology process) for each product group or product-specific.
4. A written commitment to assure food hygiene and safety for food materials and food products which the establishment produces and/or trades in (if any – following the form)
5. A lawful copy of health certificate of the establishment owner and the persons directly engaged in food production and/or trading.
6. A lawful copy certificate of food hygiene and safety training of the establishment owner and the persons directly engaged in food production and/or trading.
II. Executive time: Within 15 working days after receiving full and valid dossiers
III. Competent agencies:
1. The Health Ministry assinges to Department of food hygiene and safety in order to grant certificates for the manufacturing facilities, sales of functional food, food fortification with micronutrients, supplementary food, food additives and natural mineral water.
2. The state agencies in local are decentralized to grant certificates to high-risk food-producing and/or -trading establishments and the food is not under the Health Ministry, specifically:
a) The provincial/municipal Health Services or Preventive Medicine Center in the centrally provinces and cities (if authorized by The provincial/municipal Health Service) (hereinafter referred to as provincial level) shall grant the certificates to establishment producing and/or trading in food which are granted business license by provincial or central level; the restaurants or collective kitchen has more than 200 people; food services in industrial zones, supermarkets, markets, hospitals; tourist sites, festivals and conferences are organized and managed by provincial level; one star – class hotels and in schools from high school or more.
b) District – level People’s Committees or Preventattive Medicine Center in the provincial districts, towns and cities are authorized by the district – level Peole’s Committees (hereinafter referred to as district level) shall grant certificates to establishment producing and/or trading in food which are granted business license by district level; food outlets, canteens, restaurants, collective kitchens and hotels are not granted certificates by provincial – level; school facilities; festivals, conferences, tourist sites, markets and hospitals are organized and management.
c) Commune – level People’s Committees is authorized by the district – level People’s Committees to grant certificates to high-risk food-producing and/or-rading establishments which not subject to business registration; households and individuals produce simple packaging food, trade in fresh food which is not packed; restaurants, stalls selling prepared food to eat during the day and the markets, tourist sites, festivals and conferences are organized and managed by the commune level; primary schools, kindergartens are not under the management of their superiors.
