PROCEDURES FOR ENSURING SECURITY AND ORDER CERTIFICATE

I.  A dossier of application for  a certificate consists of: 

1. An application for certification of eligibility for security and order to do  in fields of condition business.

2. Copies (notarized, or authenticated, or certified by state agencies as prescribed) "certificate of business registration," or "certificate of registration", or "investment license " of the facility.

3. The curriculum vitae certified by the People's Committees of communes, wards, towns or agencies of the head or the legal representative people of the facility directly manage. For foreigners, Vietnamese who settled in foreign countries must have a written declaration of personal, copies of passports, entry and exit Card (when submitted copies, originals have to be presented for inspection). 

4. List of people who work in the  facility or fields of condition business of  part of the facility. 

5. Area map of the productive and business facilities (the map of rest rooms, productive areas, warehouse of materials and related areas).

6. "Certificate of eligibility on preventing and fighting fire" for the facility do business in the field of gas, flammable liquids, buildings from 11 floors or more which are used as hotels, houses, offices.

Or "Inspection records verify conditions on preventing and fighting fire" for the business field remaining.

II. Execution time: 03 working days from the date of receiving valid dossier. 

III. Agencies  implement directly administrative procedures: administrative management team of district police.
